General Information

To register for a Workshop and/or to obtain a copy of the CHQI Telemedicine Accreditation Program Standards and Guide, Version 2.1, the following information, along with payment, must be provided: Name, Title, Company Name, Address, Email, and Telephone Number.

 

Payment

CHQI accepts American Express, Visa and Mastercard. Payment is due at the time of purchase. An electronic receipt will be automatically generated.

 

Refund Policy

Payment for the CHQI Telemedicine Accreditation Program Standards and Guide, Version 2.1, is nonrefundable.

If you are no longer able to attend a Workshop, you must notify CHQI, in writing, at least seven (7) days prior to the event in order to receive a refund. Please note, an administrative fee of $75.00 will be deducted from the refund. Please send cancellation notices directly to sales@chqi.com. Processing time for a refund is 5-10 business days.

Transferability

Workshop tickets are fully transferable in date and attendee.
